Required paperwork

To ensure your athlete is eligible to participate and can be placed on a roster, all required paperwork must be completed and submitted before the season begins.

Please review the requirements below and submit all documents as soon as possible to avoid delays in participation.

Required Documents

Sports Physical

A current sports physical is required for all football and cheer participants. Physicals must be completed using the approved form and remain valid throughout the season.

Birth Certificate

A copy of the athlete's birth certificate may be required for age verification and league eligibility purposes.

Grade Verification

Athletes may be required to provide proof of grade level in accordance with league requirements.

Registration Forms

All registration information must be completed through TeamSideline, including emergency contact information and required acknowledgments.

Medical Information

Parents or guardians should provide any important medical information, allergies, medications, or special considerations during registration to help ensure athlete safety.

Submission Information

Required documents can be uploaded through TeamSideline or submitted during designated registration and paperwork events.

Families are encouraged to submit paperwork early to ensure eligibility is verified before practices and games begin.

Important Reminder

Athletes may not be permitted to participate in practices, games, camps, clinics, or other team activities until all required paperwork has been received and approved.
